Urgent Referrals

If your hematology consult is urgent, please page the on-call physician through locating (X78222).

 

General Hematology, Malignant Hematology & Bone and Marrow Transplant Referrals

A physician referral is required. Please fax your referral to (613) 737-8861.

All referrals must include:

  • Referral letter
  • Most recent bloodwork results (CBC / Biochemistry)
  • Patient demographics
  • Radiological test results, if available
  • Pathology results, if available

 * NB: if the patient has not had blood work completed within the past 6 months, please send the patient for blood work and fax the results to our office once they are available.

Once a referral is received, it is reviewed by a physician for initial triage.

  • Malignant Hematology: Referrals received Monday-Thursday are triaged within 24 hours. Referrals received on Friday will be triaged on Monday.
  • General Hematology: Referrals received Monday-Thursday are triaged within 48 hours. Referrals received on Friday will be triaged on Tuesday.

Once this initial triage has been completed, a fax will be sent to your office with the name and contact information of the physician who will be seeing your patient. Please pass this information on to your patient. At this stage, we will also advise you of any issues with your referral (e.g. wrong type of referral, missing information, required bloodwork, etc.)

After the initial triage, the physician who will be seeing your patient performs a second triage to determine priority. Patients will then be booked for an appointment based on assessed priority.

Within two weeks of receiving your referral, we will contact you with an appointment date OR an estimate, within a 2 month timeframe, of when the appointment will occur

 

Thrombosis Referrals

A physician referral is required. Please fax your referral to (613) 737-8093.

Once a referral is received, it is reviewed by a physician for triage. Patients will then be booked into the Thrombosis clinic based on assessed priority. As per hospital policy, we will contact you within 2 working days to confirm receipt of your referral, and within 2 weeks with an appointment date OR an estimate (within a 2 month timeframe) of when the appointment will occur.

If you are referring a patient for a perioperative consult, please send in your referral at least 2 weeks before the date of the planned procedure / surgery.
