Regional Geriatric Program of Eastern Ontario
The Regional Geriatric Program of Eastern Ontario (www.rgpeo.com), under the medical directorship of Dr. Frank Molnar (Division of Geriatric Medicine) and administrative directorship of Kelly Milne, manages three clinical programs that see thousands of high risk seniors per year striving to keep these seniors as independent and high functioning as possible in their homes and to thereby prevent avoidable hospitalization.
The three clinical programs are:
- The east and west Geriatric Assessment Outreach Teams (GAOT) operating out of Bruyere Continuing Care and the Queensway Carleton Hospital respectively. These teams see almost 2000 seniors in their homes annually and work with Primary Care Practitioners, CCAC and Specialized Geriatric Services to care for complex seniors at high risk of hospitalization.
- The Geriatric Emergency Management (GEM) program which operates out of emergency departments throughout the Champlain region. The GEM program sees 3500 high risk seniors annually and also works with Primary Care Practitioners, CCAC and Specialized Geriatric Services to prevent avoidable admission to hospital and to optimize function and safety.
- The Nurse led Outreach Teams (NLOT) working in Nursing Homes and liaising with Emergency Department staff to prevent unnecessary Emergency Department use and unnecessary admissions to hospital
